Head to the automatic follow-up examination reception machine and automatic insurance card authentication machine
- You will complete your reservation check-in when you insert your examination card into the machine.
- Please take your examination ticket and examination card and then wait in front of the department where you will be examined.
Those with a Reservation for a Test
Head to the test counter
- Please take your reservation ticket and head to the test counter.
Those without a Reservation
Those Who Have Received an Examination within the Past Year
Head to the automatic follow-up examination reception machine and automatic insurance card authentication machine
- The departments where you can receive examinations will be displayed. Therefore, please press the button of the department where you wish to receive an examination.
- You will be issued with a medical questionnaire coupon if you need to complete one. If you will receive your examination in the Department of Ophthalmology, please check in directly with the Department of Ophthalmology Outpatient Clinic. If you will receive your examination with another department, please check in with the outpatient clinic on each floor.
- Please take your examination ticket and examination card and then wait in front of the department where you will be examined.
Those for Whom It Has Been More Than a Year Since Your Last Examination
Head to the automatic follow-up examination reception machine and automatic insurance card authentication machine if you have an examination card
- The departments where you can receive examinations will be displayed. Therefore, please press the button of the department where you wish to receive an examination.
- You will be issued with a medical questionnaire coupon if you need to complete one. If you will receive your examination in the Department of Ophthalmology, please check in directly with the Department of Ophthalmology Outpatient Clinic. If you will receive your examination with another department, please check in with the outpatient clinic on each floor.
- Please take your examination ticket and examination card and then wait in front of the department where you will be examined.
*Please tell the initial examination reception if you have lost your examination card. We will re-issue your card for a fee (220 yen).
*You will need to pay a separate 5,500 yen (including tax) as an elective treatment fee if you do not have a letter of referral.
