Guide to Hospital Admittance


We give you information on the schedule up to your admittance to hospital and the things to bring with you when you are admitted to hospital on this page.
We give you information on your meals while in the hospital and the doctors who will examine you on this page.

Hospital Admittance Procedures

Schedule from the Hospital Admittance Reservation Procedures
to the Day of Your Hospital Admittance

  • The examining room of the outpatient clinic in each department will give you a hospital admittance application form if you will be admitted to the hospital. Please complete the procedures at the Hospital Admittance Reception on the 1F.
  • Please complete all the required items without omission on the hospital admittance information / consent form and hospital admittance guarantee. Please then sign them or stamp them with your seal. (We will give you consent forms for your tests and surgeries, so please bring them to the ward on which you will be admitted.) Please present these documents to the Hospital Admittance Reception on the day on which you are admitted to the hospital together with your insurance card and medical payment recipient card (e.g., for elderly patients and diseases specified by the government).
  • If you need a large medical fee limit certificate, please complete the procedures at the insurance association you joined before being admitted to the hospital or your municipal office.
  • If the information on your insurance card changes while you are in the hospital, please tell the Hospital Admission Reception or the Ward Clerk.

Things to Bring with You When You Are Admitted to the Hospital

  • Examination card
  • Hospital admittance consent form and hospital admittance guarantee
  • Health insurance card of the insured person (you will need to notify us when there has been a change) and other publicly funded medical cards
  • Hospital admittance treatment plan (those who were given this by the outpatient clinic when it was decided you will be admitted to the hospital)
  • Medicines you are taking (If you have been prescribed medicine by another medical institution or by an outpatient clinic at our hospital, please present the medicine you will need for your time in the hospital at the nurse station)
  • Personal belongings
    Toiletries (toothbrush, toothpaste, hairbrush, conditioner, shampoo and body soap), tissue paper, indoor footwear (non-slip footwear), earphones (excluding private rooms) and underclothes
*The above daily necessities are also sold in the convenience store on B3 in the hospital

Overview of Personal Medicines You Will Bring
When You Are Admitted to the Hospital

  • “Personal medicines” refers to medicines you take regularly (including those of other departments and other hospitals), over-the-counter medicines and supplements.
  • It is extremely important we check the impact your personal medicines will have on surgeries and tests to provide you with appropriate treatment while in hospital. Your prescription bags, medicine instructions and medical diary also contain important information. Therefore, please bring them in addition to your medicines.
  • Please bring the medicines you will need while you are in hospital.
  • The nurse station on the ward will look after all your medicines.
  • If you run out of medicine while in the hospital, please tell your attending physician or nurse in charge. We will prescribe you medicine from the hospital.
    *Please understand that you will not be able to receive an examination from another hospital or be prescribed medicine from another hospital without the permission of your doctor while in the hospital.

Other

Overview of Name Bands (Supplied by the Hospital)
Please wear a name band on your wrist while in the hospital. This is to confirm that you are a patient. Please do not bring the following items into the hospital,
  • Dangerous items (e.g., scissors, lighters, edged tools, nail clippers, vases and hairdryers)
  • Electrical appliances (e.g., TVs and electric water boilers)
  • PCs and iPads
  • Bedding
*Please ask a nurse if you have any questions.
Safety Deposit Box
Please minimize the cash and valuables you bring with you and use the personal safety deposit boxes.
Medical Counselling Room (Location: 1F Community Cooperation Room)
The people in charge here will listen to your worries about your illness and your stay in the hospital (e.g., questions about cancer, questions about medical welfare and questions about medical safety). Please feel free to ask if you have any questions.

How You Will Spend Your Time in the Hospital

Meals

  • Your attending physician will determine your type of meals according to your symptoms. We will then serve you meals suitable for your illness.
  • The meals we serve in the hospital are based on the Ministry of Health, Labour and Welfare’s Dietary Guidelines for Health Promotion. We season regular meals with a daily salt content of 10 g.
  • Please lead a healthy life based on the taste of hospital food even after you are discharged.
  • The meal times are roughly as follows: breakfast is 7:30 a.m., lunch is noon and dinner is 6 p.m.)
  • Please do not bring in food or drink from outside the hospital. Please ask your doctor or nurse if you would like to eat something other than the meals served in the hospital.
  • Only chopsticks and teacups are available in the hospital.
  • Please ask a dietician if you have any questions about the meals in the hospital.

Overview of Your Doctor for Examinations

  • Your attending physician will examine you.
  • The director of the hospital and the head of each department will make rounds regularly and give advice to your attending physician so that we can provide you with even better care.
  • The doctor on duty will provide you with care as necessary when your attending physician is absent (e.g., at night or on holidays). Please speak to a nurse in case of an emergency.

Overview of Explanations about Your Illness

  • Your attending physician will give you an explanation about your illness, tests and treatment. However, please ask if you have any questions.
  • Please choose people who you can trust (e.g., family members) as those who will receive explanations about your illness other than yourself in advance to protect your privacy in regards to your illness.
  • Please ask your attending physician or a nurse if you have any questions or requests.

How You Spend Your Time in the Hospital

  • Please follow the instructions of the doctors, nurses and other hospitals while in the hospital. We may discharge you if you do not comply with instructions.
  • Please always keep your room clean and do not bring in anything you do not need.
  • We do not allow you to bring in heaters or other electrical appliances. We do not allow you to use PCs in the four-bed rooms.
  • Please be quiet in your room so that you do not inconvenience others.
  • Please ask a doctor or nurse before using earphones to listen to the television or radio in your room. The convenience store on B3 sells earphones.
  • We turn off the lights at 9 p.m. so you can get enough sleep. We do not allow loud sounds or chatting after turning off the lights.
  • Please follow the instructions of the doctors and nurses when using the bath or shower.
  • Smoking is completely prohibited in the hospital and on the hospital grounds. Please do not smoke while you are in the hospital.
  • Please use the prepaid cards sold in the hospital to use the televisions, refrigerators and vending machines. (You can get a repayment of the balance on prepaid cards at the automatic fare adjustment machine on B3).
  • You can make a telephone call to an outside line from the one-bed rooms (private rooms) by lifting up the receiver and dialing the telephone number. You can make international telephone calls direct from the one-bed rooms (private rooms). (We will bill you for the telephone call fees together with the hospital admittance charges.)
  • There are pay phones in each ward. Please do not use them from after 9 p.m.at night to the early morning. We will not accept telephone calls from outside the hospital. However, we will inform your room that there was a telephone call.
  • Please ask a nurse or your attending physician if you would like to go out of the hospital or to go home for the night.
  • We may ask you to change rooms for treatment or ward management reasons. Please cooperate with that.
  • Please use the Cash Dispenser (CD) Corner (Yokohama Bank and Suruga Bank) on the 1F to withdraw money.
  • The convenience store on B3 sells newspapers, magazines, daily necessities, towels, luxury items and other products.
  • There is a barber shop on B3. Please use it upon asking a doctor or nurse.
  • There is a restaurant where you can have food and drinks with a view of the Bay Bridge on the 13F. Please feel free to use it.
  • Please follow the guidance of doctors, nurses and other members of staff in the event of an earthquake, fire or other emergency.
  • Please treat the facilities of the hospital and items we loan you with care. We will charge you the cost price if you damage or lose them.
  • Please do not enter or leave through other rooms, examining rooms or nurse stations.
  • Please ask a nurse if you have any questions about using your room.

Discharge Procedures and Settlement of Your Account

Overview of Hospital Admittance Charges Payment and Discharge

Payment hours 9:30 a.m. to 5 p.m. (Including public holidays)
  • We will calculate your hospital admittance charges at the end of each month. We will then deliver an invoice to your room by around the 10th of the following month. We will deliver an estimate to your room the day before you are discharged.
  • Credit card payment: We accept AMEX, DinersClub, DISCOVER, JCB, DC, VISA, MasterCard and UFJ Card. Please ask at the Hospital Admittance & Discharge Accounting Counter on the 1F.
  • We will invoice you at the Hospital Admittance & Discharge Accounting Counter on the 1F on the day you are discharged. Therefore, please pay directly at the Hospital Admittance & Discharge Accounting Counter from 9:30 a.m.
    *We will calculate your invoice with the Advance Hospital Admission & Discharge Charges Payment Deposit Receipt. Therefore, please do not forget to bring it with you.
  • Please ask the person in charge of hospital admittance in the Medical Affairs Section before paying if you have any questions.
  • You will need your receipt for income tax medical expense deduction declarations and high-cost medical expense applications. Therefore, please keep it in a safe place. Please note that we will not reissue it to you.
  • We will give you the necessary items when you are discharged (e.g., examination card and medicines) at the ward.
  • Please reserve your next outpatient clinic day at the Outpatient Clinic Block Reception if you will need another examination.
  • Please ask the Documents Clerk at the General Reception on the 1F if you require a medical certificate after you are discharged.

For Visitors

Introduction

  • Please see the “Transportation Access” page for maps to Keiyu Hospital and bus times. (Please click here for information on the parking lot.)
  • Keiyu Hospital has facilities visitors can use (e.g., Flower Hanami and a tearoom).

Rules We Want You to Observe

Weekdays Saturdays, Public Holidays and National Holidays
General wards 1 p.m. to 5 p.m. 1 p.m. to 5 p.m.
Neonatal Room 3:15 p.m. to 4:15 p.m. 3:15 p.m. to 4:15 p.m.
  • We have determined the above visiting hours in consideration of the treatment and rest of patients.
  • If you are visiting a patient, please complete a visit application card at the nurse station in each ward and then wear it. Please complete the time you leave the hospital on the visit card and then return it to the Visit Card Return Box when you leave the hospital after your visit.
  • Please do not inconvenience other patients during your visit. Please do not visit patients in large numbers or with small children. This is to prevent the interference of patient’s rest and infections.
  • Visitors should please not eat or drink in the patient’s room.
  • Please use the spacious dayroom or the dayroom with a view of the harbor for your visits.
  • Please tell the nurse station in the ward if you would like a visit outside of the designated hours due to special circumstances.
  • Please do not visit the ward in the Department of Pediatrics with children of elementary school age or younger.
  • Please do not visit the ward in the Department of Obstetrics (including the 6F and part of the 7F) with children of elementary school age or younger.

Room Introduction

Guide to the Special Rooms on the 12F

These rooms come with a bath, toilet and shower in a spacious 36 m2 space.

The rooms are also replete with the other facilities you will need to spend your time in comfort: reception room, washroom, electric bed, feather quilt, telephone (fee required for calls), TV and refrigerator (free).

In addition, the Minatomirai Bay Bridge you can see from the window looks truly beautiful on a nice day. It will lift your spirits.

Private Room Information

Our private rooms have a space of 18 m2. You can spend your time in them comfortably while enjoying the view from your window because they come with the facilities below.

Four-bed Room Information

These rooms have four beds in a space of 36 m2. You can see the sea of Minatomirai from the four-bed rooms.

Guide to the Four-bed Rooms in the Department of Obstetrics on the 6F

These rooms have four beds divided by partitions in a space of 36 m2. They come with the facilities below. Newspapers are placed in these rooms every morning.
